Wertz suggests that we will probably continue to treat clients with aphasia in the early postonset period. I hope he’s right and that my view of managed care as a runaway train is an overreaction. The comments and feedback from clinicians that I have talked with suggest otherwise (Marshall, 1994, 1995). Managed care is not coming; it’s already here. In the current climate, the worse decision may be no decision. In the absence of the proof that Wertz rightfully calls for, we need to make the best of what we have.
